Patient Portal:  This tool allows you to fill out all the required paperwork online prior to your vist. You can request an appointment, request a refill, ask the doctor a question about your care, access your lab/pathology results and, in some cases, your medical record. Please call for your personal password and then click on the patient portal link on the upper left of this page and follow the directions from there.

Prescription Refill Requests: Make a request through the patient portal (button on the upper left of this page) and request your refill. You can also leave a message on the office phone. Your prescription will be refilled and you will be contacted within 48 hours. In general, if you have not been seen in the last year, your prescription cannot be refilled.

Schedule and Appointment:  If you are not an established patient, please call 801.763.7101.  If you are an established patient and have access to the patient portal, you can request an appointment online.

After Hours:  For EMERGENCIES ONLY: Call the office number: 801.763.7107. The phones will forward to the doctor after hours.  Prescriptions will not be refilled through this number.  Otherwise, please send a message through the patient portal and you will be contacted within one business day.  You can also leave a message on the office phone and it will be returned within 48 hours.

Electronic Medical Record:  Alpine Dermatology, PC uses an electronic medical record called Prognocis.  This allows you to access your results in a timely manner and allows Dr. Eberting to access your record and results remotely.  We offer e-prescribing which allows us to send your prescriptions directly to your pharmacy if desired.  You must provide the pharmacy fax and phone number.  For a yearly fee of $25 you can order access to your entire electronic medical record so you or your physicians can access it anytime from anywhere in the world.

Insurance Policy:  Alpine Dermatology, PC will provide care to you even if you do not have insurance.  We will also see you if you have a policy with which Dr. Eberting is not contracted.  For such patients, we follow a fee for service model.  You will be offered a cash discount from the standard billed rates.  We will also provide you with a billing form that you can mail to your insurance company for reimbursement.  Alpine Dermatologyis currently contracted with the following plans:

• Medicare

• Select Health (We take Select Med, Select Care, and Select Plus--we do not take Select Value). 
• We are in the process of credentialing with Beech Street plans.
